Named GP Policy

Purpose

The purpose of this policy is to set out the arrangements by which every patient is assigned a named GP, in order to ensure continuity of care, improved patient outcomes, and a stronger doctor–patient relationship.


Background

Continuity of care is associated with better health outcomes, improved patient satisfaction, and more efficient use of healthcare resources. NHS England requires all GP practices to allocate a named accountable GP to each patient, particularly those aged 75 and over. This policy extends that principle to all registered patients.


Scope

This policy applies to:

  • All registered patients at the practice
  • All clinical and administrative staff
  • Locum and temporary staff, where applicable

Policy Statement

Each patient registered at the Barcellos Family Practice will be assigned a named GP who is responsible for:

  • Overseeing the patient’s overall care within the practice
  • Taking lead responsibility for the coordination of health and social care services
  • Supporting continuity of care, particularly for patients with complex, long-term conditions

Responsibilities of the Named GP

While the named GP does not need to provide all care personally, they are responsible for:

  • Ensuring that the patient’s care is coordinated across clinical teams
  • Reviewing test results, hospital correspondence, and clinical records where appropriate
  • Being a point of contact for multidisciplinary teams, if required
  • Participating in annual reviews or care planning discussions for patients with long-term conditions

Communication of Named GP to Patients

  • Patients will be informed of their named GP upon registration and during contact with the practice.
  • The named GP will be printed on correspondence and visible in the patient’s electronic medical record.
  • Patients may request a change of named GP, which the practice will consider based on availability and clinical appropriateness.

Patient Access and Appointments

  • Patients may continue to see any GP of their choice. Having a named GP does not restrict access to other clinicians.
  • The practice will, where possible, facilitate appointments with the named GP to support continuity.

Allocation Process

  • Patients will be allocated a named GP based on current registration lists and clinical capacity.
  • The administration team who complete the registrations will oversee regular audits to ensure all patients have a named GP recorded.

Monitoring and Review

  • The practice will monitor compliance with this policy through periodic audits.
  • The policy will be reviewed annually or in line with changes in national guidance.

Equality and Diversity

The policy applies equally to all patients and staff. No one shall be treated unfairly or discriminated against based on age, sex, race, disability, sexual orientation, religion or belief.
